Phoebe Sumter Medical Center, a brand-new medical facility located at 1048 East Forsyth Street (site of the former HealthPlex)in Americus, will be home for all former Phoebe Sumter Medical Center patients until our state-of-the-art, permanent hospital opens in a few years. Until then, patients will enjoy upgrades and improvements never before offered in our community.

All 76 beds in this brand new 71,000 square-foot facility are located in private rooms. Each department - Labor & Delivery, Emergency, Radiology, Surgery and Intensive Care, offers patients the best new, 21st century diagnostic and testing equipment available in the health care industry. Behaviorial health and the Phoebe Sumter Medical Center sleep lab are the only departments that will not transfer from Phoebe Sumter Medical Center to the interim hospital.

Patients will also enjoy convenient, quality care from the medical professionals they have come to know and trust over the years. Patient Care
Phoebe Home Care provides health care for all ages in the convenience of your home. We accept Medicare, Medicaid, private insurance and VA benefits. We service the following counties: Sumter, Randolph, Terrell, Lee, Calhoun, Dougherty, Worth, Early, Baker and Miller.

Guidelines Specific to Medicare
In order to receive home health services, the patient must be homebound. A homebound patient is defined as one who requires considerable effort and assistance to leave home. Medicare guidelines require that the care given must be medically reasonable, necessary and ordered by a physician. The patient must require skilled care that can be administered intermittently.

Skilled Nursing Care
Our skilled staff of RNs and LPNs provide home care comparable to the services offered in the hospital.

Assess blood pressure, pulse and respiratory
status
Monitor nutrition, hydration, bowel and bladder
functions
Monitor and teach medication regime and response
to medications
Diabetic teaching - check glucose levels and
administer insulin
Diet instruction under guidance of a dietitian
Wound care changes - urethral or suprapubic
Colostomy/ileostomy care
Draw blood samples - venous-arterial
IV therapy, continuous antibiotics/TPN enteral
feeding

Physical Therapy
A patient’s deterioration, caused by inactivity, can usually be alleviated through the rehabilitation services of a qualified physical therapist working in the home.

Gait training
Bed mobility transfers
Strengthening and therapeutic exercises
Proper application and use of prosthetic devices
Speech Therapy

Cancer, stroke and disease of the aged can often impair the ability to speak clearly. A speech therapist, working in your home, can help the patient overcome these problems.

Language and communication retraining
Dysphasia

Occupational Therapy (Limited availability)
An impairment of function to perform daily activities such as grooming, dressing, preparing and eating meals may result from stroke, severe arthritis or injury. In the home, an occupational therapist can assist a patient to regain these skills.

Home Health Aide Service
An aide provides the same services received by a patient in the hospital or nursing home, plus additional services required at home, such as meal preparation and light housekeeping.
